Nomination Guidance

Nominations will be asked to respond to the following questions, or provide appropriate evidence. Those making a nomination are advised to prepare their responses in a separate document first, and copy these into their application when ready.

Excellence in Care Award

  1. Provide details about how the nominee works with a team approach to ensure great patient care (200 words)

  2. How does the nominee go above and beyond in providing excellent service to patients and the community? (200 words)

  3. Provide examples of excellent patient outcomes. (200 words)

  4. Why does the nominee deserve to receive a Dental Care Award in this category? (200 words)

  5. Upload two testimonials in support of this nomination. Testimonials should be from professional colleagues, or in the case of a practice nominee, from external stakeholders (suppliers or other business owners in the community) Patients should not be asked to provide testimonials

  6. For practice nominees only - please explain how you attract patients to your service in a way that utilises ethical marketing practises and does not exploit patients.

Outstanding Young Dental Professional Award

  1. Detail the nominee's achievements to date (200 words)

  2. What makes you feel that the nominee is different to other young dental professionals? (200 words)

  3. How does the nominee demonstrate that they go over and above their duty of care in the provision of care? (200 words)

  4. How does the nominee contribute to the wider dental profession? (200 words)

Enhancing Access to Dental Care Award

  1. Detail how the nominee contributes to making oral healthcare more accessible? (400 words)

  2. Describe how the nominee ensures appropriate clinical governance in their work (200 words)

  3. Provide two colleague testimonials in support of this nomination. Testimonials should address how the nominee’s work has actively promoted access to dental care.

Promoting Professionalism in Dentistry Award

  1. Explain how the nominee demonstrates commitment to professionalism and how they have promoted the professionalism of others. (400 words)

  2. Describe a time where the nominee's professionalism was significantly challenged and detail how they navigated this. (200 words)

  3. Has the nominee ever had professional regulatory action taken against them? If yes, please provide details.

  4. Describe the nominee's approach to managing a patient or service user complaint - please provide a de-identified example of a real case where possible. (200 words)

  5. If the nominee works in a commercial environment, how do they manage marketing in a professional and ethical manner (200 words)

Enhancing the Mission of Dentistry Award

  1. Describe the nominee's contribution to the professional mission of dentistry. In your response, please provide comment on:

    • How the nominee has improved oral health beyond clinical care

    • What charitable activities relating to oral health has the nominee been involved in? 

    • How does the nominee display altruism in their work? 

    • Describe clear outcomes that the nominee has been able to achieve that relate to furthering the collective goals of organised dentistry?

Outstanding Contribution to Public Dentistry Award

  1. Provide a brief overview of how the nominee organisation contributes to the mission of the public oral health services (500 words)

  2. Articuate any priority population or specific service need that the organisation contributes to providing care for (200 words)

  3. Upload any additional evidence that supports the organisation’s nomination in relation to work in the advancement of public dentistry

Exceptional Student Award

  1. Why does the nominee deserve to be recognised?

Outstanding Dental Technician Award

  1. Detail how the nominee provides outstanding work as a dental technician (200 words)

  2. Detail the training and education of the nominee. (200 words)

  3. Provide details of the nominee's career to date (200 words)

  4. Describe the current services the nominee provides (200 words)

  5. Provide evidence of three professional cases where the dental technician has been the lead contributor to the final prosthesis or appliance

  6. Provide references from three clinicians who the nominee supports in providing dental laboratory services.

Championing the Patient Journey

  1. Describe how the nominee promotes a culture of service within the care environment (200 words)

  2. Outline an initiative or program that the nominee has established or actively contributed to that has enhanced the patient journey and experience in their work environment (400 words)

  3. How has the nominee advanced the engagement and involvement of patients and their families within their care experience? (200 words)

  4. Upload two statements in support for this nomination. These may be from:
